What is Adrenal Adenomectomy?

Adrenal adenomectomy is the surgical removal of benign (adenoma) masses of the adrenal gland. Hormonally active adenomas (aldosteronoma, cortisol-producing, pheochromocytoma) or nonfunctional adenomas 4 cm or larger are usually surgical candidates.

Laparoscopic adrenalectomy has become the gold standard, providing less pain, early mobilization, and short hospital stay. The retroperitoneoscopic technique is widely used in experienced centers.

Preoperative endocrine evaluation, optimization of hypertension and electrolyte balance, and alpha blocker preparation in suspected pheochromocytoma are critical.

Symptoms

Uncontrolled hypertension
Hypokalemia and muscle weakness (Conn syndrome)
Cushing features (monthly weight gain, purple striae)
Headache, palpitations, sweating (pheochromocytoma)
Hirsutism and menstrual irregularities
Incidentaloma — incidental mass detection
Weight loss and weakness (suspected malignancy)

Risk Factors

Genetic syndromes such as MEN2 and von Hippel-Lindau
Family history of pheochromocytoma
Primary hyperaldosteronism
History of Cushing syndrome
Detection of incidentaloma
Female sex (in nonfunctional adenomas)
Middle and older age

When to See a Doctor?

  • Resistant hypertension and electrolyte disturbance
  • Paroxysmal attacks raising suspicion of pheochromocytoma
  • Findings of Cushing syndrome
  • Mass over 4 cm seen on imaging
  • Individuals requiring incidentaloma follow-up
