What is Acquired Cancer Resistance Mechanisms?

Acquired cancer resistance refers to tumor progression after an initial response to treatment, driven by genetic, epigenetic, and microenvironmental changes that emerge under therapeutic pressure. It is now recognized as the rule rather than the exception in advanced cancer.

Mechanisms include secondary mutations in the drug target, activation of bypass signaling pathways, gene amplification, drug efflux, lineage plasticity such as small-cell transformation in lung cancer, and immune escape via downregulation of HLA or interferon signaling.

Modern oncology relies on serial molecular profiling, including liquid biopsy of circulating tumor DNA, to detect resistance early and select the next therapy. Rational drug combinations and adaptive trial designs aim to delay or prevent resistance.

Symptoms

Initial tumor shrinkage followed by regrowth or new lesions
Rising tumor markers despite ongoing therapy
New imaging changes in previously controlled disease
Need for repeat biopsy or liquid biopsy
Decision point for next-line systemic therapy

Long duration of single-agent therapy
High initial tumor mutational burden
Insufficient drug exposure or non-adherence
