What is Abdominal Pain (Differential Diagnosis)?

Abdominal pain is a complex symptom reflected by more than one organ system. The stomach, small and large intestine, liver, gallbladder, pancreas, kidneys, and reproductive organs in the abdominal cavity, as well as adjacent structures such as the heart and lungs, can produce pain that may be felt as abdominal pain.

The location, character (sharp, cramping, burning), onset (sudden/gradual), duration, and accompanying symptoms (fever, nausea, jaundice, blood) of the pain are critical elements that guide diagnosis. For example, right lower quadrant pain suggests appendicitis, right upper quadrant pain suggests gallbladder pathology, and epigastric pain radiating to the back suggests pancreatitis.

Abdominal pain that is sudden, severe, or accompanied by serious symptoms requires emergency evaluation; in 'surgical abdomen' presentations (appendicitis, perforated ulcer, bowel obstruction), delay can be life-threatening.

Symptoms

Pain localization suggesting different source organs depending on the area
Fever (infectious or inflammatory cause)
Nausea, vomiting, or loss of appetite
Jaundice (liver or biliary tract)
Blood or burning in urine (urinary system)
Blood or color change in stool
Abdominal tenderness and rigidity

Risk Factors

Previous abdominal surgery
History of chronic gastrointestinal disease
Excessive alcohol consumption
Presence of gallstones or kidney stones
High-dose NSAID use
Immunosuppressive therapy
Family history (colorectal cancer, inflammatory bowel disease)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Sudden-onset and progressively worsening severe abdominal pain (emergency!)
  • Abdominal rigidity, board-like abdomen (suggesting perforation)
  • If fever and jaundice occur together
  • If accompanied by blood in stool or bloody vomit
  • Pain accompanied by altered consciousness or fainting
