What is Vitreous Haemorrhage?

Vitreous haemorrhage is extravasation of blood from retinal or uveal vessels into the vitreous cavity.

Proliferative diabetic retinopathy is the commonest cause, followed by retinal tear after posterior vitreous detachment, retinal vein occlusion, trauma, and Terson syndrome (subarachnoid haemorrhage).

Presentation is sudden painless vision loss, dense floaters, or reddish shadowing of vision.

B-scan ultrasonography is essential when the retina cannot be visualised to exclude retinal detachment.

Symptoms

Sudden vision loss or significant blurring
Dense floaters or cobwebs
Reddish or dark tint to vision
Painless visual decline
Flashes of light if accompanied by retinal tear

Risk Factors

Proliferative diabetic retinopathy (most common)
Retinal vein occlusions
Posterior vitreous detachment with retinal tear
Ocular trauma
Anticoagulant or antiplatelet therapy
Age-related macular degeneration (neovascular)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If sudden painless vision loss develops
  • If dense floaters appear suddenly
  • If flashes of light and floaters occur together (retinal tear suspected)

Treatment Methods

Bed rest with head elevation to allow blood to settle

B-scan ultrasonography to assess for retinal detachment

Treatment of underlying cause (pan-retinal photocoagulation for diabetic retinopathy)

Pars plana vitrectomy if blood does not clear or retinal detachment is present

Anti-VEGF therapy for neovascularisation

Discontinuation or adjustment of anticoagulants when appropriate
