What is Branch Retinal Vein Occlusion (BRVO)?

BRVO results from thrombotic occlusion at an arteriovenous crossing where the rigid retinal artery compresses the underlying vein.

Hypertension, diabetes mellitus, hyperlipidaemia, and glaucoma are the principal systemic risk factors.

Downstream territory shows intraretinal haemorrhages, cotton-wool spots, and macular oedema causing vision loss.

Anti-VEGF intravitreal therapy and sectoral laser photocoagulation are the evidence-based mainstays of treatment.

Symptoms

Sudden or progressive painless vision loss in one eye
Blurred area or shadow in part of the visual field
Metamorphopsia (straight lines appear wavy)
Floaters
No eye pain or redness

Risk Factors

Hypertension (most common systemic association)
Diabetes mellitus and dyslipidaemia
Primary open-angle glaucoma or ocular hypertension
Age over 50 years
Hyperviscosity syndromes and thrombophilia

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If sudden vision loss develops in one eye
  • If a dark area appears in the visual field
  • If straight lines appear bent or wavy

Treatment Methods

01
Intravitreal anti-VEGF injections (ranibizumab, aflibercept, bevacizumab)
02
Sectoral scatter laser photocoagulation for non-perfused retina
03
Intravitreal dexamethasone implant for persistent macular oedema
04
Strict control of systemic risk factors (blood pressure, glucose, lipids)
05
Close follow-up for neovascular complications and vitreous haemorrhage
