What is Scleritis?

Scleritis is a deep and potentially destructive inflammation of the sclera, the outer protective layer of the eye. It is classified as anterior (diffuse, nodular, necrotizing) and posterior. Necrotizing scleritis is the most severe form and can lead to scleral melting and perforation. In about 50% of cases, systemic autoimmune diseases such as rheumatoid arthritis, granulomatous polyangiitis, and relapsing polychondritis accompany it.

Symptoms

Severe, deep, dull eye pain
Dark red-purple redness in the eye
Pain worsening at night and waking the patient from sleep
Light sensitivity and tearing
Decreased vision (especially in the posterior type)

Risk Factors

Rheumatoid arthritis and other autoimmune diseases
Granulomatous polyangiitis (Wegener)
Relapsing polychondritis
Infectious causes (herpes zoster, tuberculosis)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If severe eye pain has developed
  • If there is dark red-purple redness in the eye
  • If blurred vision accompanies the symptoms

Treatment Methods

01
Oral NSAIDs (in mild cases)
02
Systemic corticosteroids
03
Immunosuppressive drugs (methotrexate, azathioprine)
04
Biological agents (in resistant cases)
05
Scleral graft (if there is a perforation risk in necrotizing form)
