What is Systematic Chest Radiograph Interpretation?

Systematic interpretation begins with image quality assessment (penetration, rotation, inspiration), then proceeds through airway and trachea, bones and soft tissues, cardiac silhouette, diaphragm, edges/effusions, lung fields and finally devices/lines/tubes.

Pattern recognition includes alveolar (consolidation, edema), interstitial (reticular, nodular, honeycomb), atelectasis (subsegmental, lobar, complete), mediastinal masses, pleural disease and pneumothorax/pneumomediastinum.

Common pitfalls include missing lung apices, retrocardiac region, costophrenic angles, behind the diaphragm and mediastinal-soft-tissue lines; comparison with prior films and integration with CT and clinical context are essential.

Symptoms

Cough, dyspnea or chest pain prompting imaging
Trauma chest with hypoxia
Suspected pneumonia or pleural effusion
Heart failure assessment
Pre-operative or screening evaluation

Risk Factors

Frequent chest imaging in acute care
Inexperience with subtle findings
High volume reads without breaks
Distractions in emergency settings
Missing prior comparison films
