What is Pulmonary Lymphangitic Carcinomatosis?

Pulmonary lymphangitic carcinomatosis is a condition that develops when an extrapulmonary or pulmonary primary malignancy spreads to the lung lymphatic channels. Breast, lung, gastric, pancreatic, and prostate cancers are the most common causes.

Accumulation of tumor cells in the lymphatics disrupts lymphatic flow and leads to interstitial edema, fibrosis, and the development of pulmonary hypertension. This process causes rapidly progressive dyspnea.

The diagnosis is supported by high-resolution CT; intralobular septal thickening and a nodular pattern are characteristic. Definitive diagnosis is made by lung biopsy. Treatment is directed at the underlying malignancy.
