What is Primary Spontaneous Pneumothorax Management?

Primary spontaneous pneumothorax (PSP) develops in individuals without underlying lung disease due to air leak from apical blebs. It is common in tall, thin young men.

According to BTS guidelines, needle aspiration is preferred in symptomatic small PSP and tube thoracostomy in large or symptomatic PSP. Recurrence risk after the first episode is 30-50%.

Symptoms

Sudden-onset pleuritic chest pain
Dyspnea
Decreased breath sounds on affected side
Tachycardia
Tension pneumothorax: hypotension and tracheal deviation - URGENT

Risk Factors

Tall, slim body habitus
Male sex (6:1)
Smoking (dose-dependent risk)
Marfan syndrome
Family history of pneumothorax

When to See a Doctor?

  • Sudden chest pain and dyspnea - urgent evaluation
  • Recurrent symptoms in a patient with previous pneumothorax
  • Suspicion of tension pneumothorax - life-saving intervention!

Treatment Methods

Small PSP (<2 cm): observation or needle aspiration

Large PSP (>=2 cm): tube thoracostomy

Tension pneumothorax: emergent needle decompression + chest tube

Second episode: surgical pleurodesis (via VATS)

Ipsilateral episode: VATS bullectomy + pleurodesis

Smoking cessation (reduces recurrence risk by 50%)
