What is Pleural Thickening?

Pleural thickening is the thickening of the visceral or parietal pleura with fibrous tissue due to recurrent inflammation, infection, bleeding, or asbestos exposure. Calcification may accompany (fibrothorax).

Tuberculous pleurisy, empyema, hemothorax, asbestos exposure, and previous chest surgery are the main causes. Mild cases are asymptomatic, while severe bilateral thickening can significantly restrict respiratory capacity.

Thoracic CT is the gold standard in diagnosis and assessment of extent. It requires careful follow-up as it may be confused with mesothelioma.

Symptoms

Often asymptomatic
Shortness of breath (in widespread thickening)
Sensation of chest tightness
Exercise intolerance
Cough (depending on accompanying pathology)
Chest pain (rare)

Risk Factors

Asbestos exposure
Previous tuberculous pleurisy or empyema
Hemothorax or recurrent pleural effusion
History of chest surgery or radiotherapy
Rheumatoid arthritis (pleural involvement)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Regular thoracic CT follow-up in history of asbestos exposure
  • If shortness of breath or exercise intolerance develops
  • To exclude mesothelioma or malignant pleural disease
  • Measurement of respiratory restriction by spirometry
