What is Obstructive Sleep Apnea Syndrome?

Obstructive sleep apnea syndrome (OSAS) is a sleep disorder featuring episodes of breathing cessation (apnea) or shallowing (hypopnea) due to repetitive obstruction of the pharyngeal airway as the upper airway muscles relax during sleep.

Each apnea episode causes brief drops in oxygenation, sleep fragmentation, and activation of the sympathetic nervous system. This is an independent risk factor for hypertension, coronary disease, stroke, and atrial fibrillation.

The gold standard diagnostic method is polysomnography (sleep test). The apnea-hypopnea index (AHI) determines disease severity. CPAP therapy substantially reduces symptoms and cardiovascular risk.

Symptoms

Severe snoring
Excessive daytime sleepiness and fatigue
Witnessed nocturnal breathing pauses
Morning headache and dry mouth
Concentration and memory difficulties
Waking up tired in the morning (feeling unrested)

Risk Factors

Obesity and large neck circumference
Male gender and advanced age
Anatomic factors: small jaw, large tonsils
Alcohol and sedative drug use
Family history of OSAS

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If breathing pauses are observed at night by partner
  • If severe snoring and daytime sleepiness are both present
  • In the presence of undiagnosed hypertension or cardiac arrhythmia
  • Visit a sleep clinic for polysomnography
