What is Lung Abscess Treatment?

Lung abscess is an infection characterized by necrosis of pulmonary parenchyma and a purulent collection. Primary abscesses arise from aspiration with anaerobes as the most common pathogens. Secondary abscesses develop from bronchial obstruction or hematogenous spread.

Abscesses larger than 4-6 cm, those unresponsive to antibiotic therapy, and abscesses in immunocompromised patients may require interventional drainage. Treatment usually lasts 4-6 weeks.

Symptoms

Fever and chills
Productive cough (purulent, foul-smelling sputum)
Chest pain
Night sweats
Weight loss
Hemoptysis

Risk Factors

Aspiration risk (loss of consciousness, alcoholism, dysphagia)
Poor oral hygiene (periodontal disease)
Immunosuppression
Bronchial obstruction (tumor)
IV drug use (septic emboli)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Fever, productive cough, and cavitation on chest radiograph
  • No response to antibiotic therapy after 5-7 days
  • Massive hemoptysis (>200 mL/24 hours)
  • When empyema develops

Treatment Methods

Empiric antibiotics: ampicillin-sulbactam or clindamycin + cephalosporin

Treatment duration 4-6 weeks (until radiologic resolution)

Percutaneous drainage (>4-6 cm or unresponsive abscess)

Bronchoscopy (to exclude an obstructive cause)

Surgical resection (refractory abscess, massive hemoptysis, malignancy suspicion)

Reduction of underlying aspiration risk
