What is Long-Term Oxygen Therapy (LTOT)?

Long-term oxygen therapy (LTOT) involves the use of oxygen for at least 15 hours a day in patients with chronic hypoxemic respiratory failure. It is one of the few therapies proven to improve survival in COPD.

It is indicated in patients with PaO2 <=55 mmHg or SaO2 <=88%. In the presence of cor pulmonale, polycythemia, or pulmonary hypertension, it can also be initiated at PaO2 <=60 mmHg.

Symptoms

Dyspnea at rest
Exercise intolerance
Cyanosis
Signs of cor pulmonale (peripheral edema)
Polycythemia (high hemoglobin)

Risk Factors

COPD (severe stage)
Interstitial lung diseases
Pulmonary vascular diseases
Neuromuscular diseases
Cystic fibrosis

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • PaO2 <=55 mmHg or SpO2 <=88% (2 measurements while stable)
  • Cor pulmonale + PaO2 <=60 mmHg
  • Polycythemia (Hct >55%) + PaO2 <=60 mmHg
  • Evaluation of need for ambulatory oxygen
