What is Goodpasture Syndrome?

Goodpasture syndrome (anti-GBM disease) is an autoimmune disease in which antibodies against type IV collagen of basement membranes damage both alveoli and glomeruli. Classically, it presents with both alveolar hemorrhage and rapidly progressive glomerulonephritis.

The disease has two peaks: in young men in their 20s-30s (pulmonary-renal form) and in elderly patients in their 60s-70s (isolated renal form). Hemoptysis and rapidly progressive renal failure are typical findings.

Diagnosis is made by demonstrating anti-GBM antibodies in serum and linear immunofluorescence staining on kidney biopsy. Plasmapheresis, immunosuppression, and rapid diagnosis are critical for survival and renal function.
