What is Eosinophilic Lung Diseases?

Eosinophilic lung diseases are a heterogeneous group of disorders characterized by an increase in eosinophil count in lung tissue and/or in blood. Simple pulmonary eosinophilia (Löffler syndrome), acute eosinophilic pneumonia, chronic eosinophilic pneumonia, and hypereosinophilic syndrome are the main forms.

Etiology includes parasitic infections, drug reactions, ABPA, and unknown causes (idiopathic). The acute form requires urgent corticosteroid therapy, whereas the chronic form progresses more slowly.

Blood eosinophil count, IgE level, sputum, and BAL (bronchoalveolar lavage) help in diagnosis.

Symptoms

Cough and dyspnea
Fever
Night sweats and weight loss (in chronic form)
Rapidly developing respiratory failure in acute form
Wheezing and chest tightness
Elevated eosinophil count in blood

Risk Factors

History of parasitic infection
New medication use (NSAID, antibiotics)
Travel to tropical regions
Atopy and asthma
Idiopathic (cases of unknown cause)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When eosinophilia is detected in blood, lung symptoms should be investigated
  • If eosinophilia accompanies acute dyspnea and fever, go to the emergency department
  • When a new drug-related reaction is suspected
  • Specialist referral for BAL and bronchoscopy evaluation

Treatment Methods

01
Oral or IV corticosteroids (first-line treatment)
02
Treatment of trigger drug or parasitic etiology if present
03
IL-5 antagonists (mepolizumab — in chronic and refractory cases)
04
Pulmonary rehabilitation and follow-up
05
Long-term low-dose steroid for relapses
