What is Drug-Induced Pulmonary Toxicity?

Drug-induced pulmonary toxicity is damage to the lung parenchyma, airways, pleura or pulmonary vasculature caused by therapeutic medications. More than 350 drugs are known to cause pulmonary toxicity.

Amiodarone, methotrexate, nitrofurantoin, bleomycin, busulfan and immunotherapy agents are most often implicated. Mechanisms include direct cytotoxic injury, immune-mediated reactions and oxidative stress.

Clinical presentations are varied: interstitial pneumonitis, organizing pneumonia, eosinophilic pneumonia, diffuse alveolar hemorrhage, bronchiolitis obliterans and pleural effusion are among the patterns seen.

Symptoms

New or worsening dyspnea
Dry cough
Fever and impaired general condition
Hypoxemia
Chest pain (pleuritic in pleural involvement)
Hemoptysis (in alveolar hemorrhage)

Risk Factors

Cumulative drug dose (bleomycin, amiodarone)
Concurrent radiotherapy (potentiates bleomycin toxicity)
High-concentration oxygen exposure
Advanced age and pre-existing lung disease
Renal failure (drug accumulation)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When respiratory complaints develop after starting a new drug
  • When dyspnea and cough occur during chemotherapy or immunotherapy
  • When new radiologic findings appear in a patient on amiodarone
