What is Diffuse Alveolar Hemorrhage?

Diffuse alveolar hemorrhage (DAH) is a syndrome characterized by bleeding from the capillaries and small vessels in the pulmonary microcirculation into the alveoli. Vasculitides, Goodpasture's syndrome, connective tissue diseases, drugs, and coagulopathies are the most common causes.

Clinically, hemoptysis, anemia, hypoxemia, and diffuse infiltrations on radiology are seen. However, hemoptysis may not occur in approximately one-third of patients. Progressively bloody fluid in sequential BAL lavages is diagnostic.

Early diagnosis and identification of the cause are essential. Treatment varies depending on the underlying cause; high-dose steroids and immunosuppressive therapy are used in immune-mediated causes.

Antiphospholipid syndrome
Bone marrow transplantation
Drug use (propylthiouracil, cocaine)
Coagulopathy and anticoagulant use
