What is Cryptogenic Organizing Pneumonia (COP)?

Cryptogenic organizing pneumonia (COP), formerly known as bronchiolitis obliterans organizing pneumonia (BOOP), is a lung disease of unknown cause. It is characterized by granulation tissue plugs in the lumen of bronchioles and alveoli.

Patients typically present with a 4-8 week history of cough, fever, and dyspnea, often treated as pneumonia, but they do not respond to antibiotics. On CT, subpleural and peribronchial consolidations are characteristic.

Diagnosis is made by combining clinical, radiological, and histopathological data. Corticosteroids are the gold standard treatment with dramatic response, although recurrences are common.

Symptoms

Subacute cough
Fever and night sweats
Dyspnea
Fatigue and malaise
Weight loss
Chest pain
Lack of response to antibiotic therapy

Risk Factors

Middle age
Connective tissue diseases
Drug exposure
History of radiotherapy
Bone marrow transplantation
Inflammatory bowel disease
Unproven viral infections

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Pneumonia not responding to antibiotic therapy
  • Prolonged cough and weight loss
  • Persistent infiltrates on chest X-ray
  • Unexplained dyspnea

Treatment Methods

01
Corticosteroid therapy (6-12 months duration)
02
Clinical and radiological monitoring
03
Extension of treatment in case of recurrence
04
Immunosuppressive drugs (in resistant cases)
05
Vaccination and infection prevention
06
Investigation of underlying causes
