What is Chronic Cough?

Cough is a physiological defense mechanism that clears the airways of irritating substances. Lasting more than eight weeks is considered chronic. Chronic cough can significantly impair quality of life; it may lead to hoarseness, fatigue, and social embarrassment.

Upper airway cough syndrome (postnasal drip), asthma/eosinophilic cough, and gastroesophageal reflux disease (GERD) are the three main causes of chronic cough in adults. Smoking, ACE inhibitor medications, and lung cancer are also among the causes that must be ruled out.

When the underlying cause is identified and treated, the cough usually resolves completely.

Symptoms

Cough lasting more than eight weeks
Worsening at night or when lying down (reflux or asthma)
Cough with postnasal drip or need to clear the throat
Sputum: color and amount guide diagnosis
Cough triggered after swallowing (reflux origin)
Cough that varies with environment and exercise (asthma)
Coughing up blood or weight loss (cancer must be investigated)

Risk Factors

Smoking
Use of ACE inhibitor medications
Allergic rhinitis and chronic sinusitis
Obesity (increases reflux risk)
Asthma or eosinophilic airway disease
Occupational irritant exposure
Continuing COPD after smoking cessation

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If cough lasts more than eight weeks
  • If cough is accompanied by blood, weight loss, or night sweats
  • If respiratory distress or high fever is present
  • For medication change if cough started while on ACE inhibitor
  • For investigation of chronic cough not responding to empirical treatment
