Atypical Pneumonia

What is Atypical Pneumonia?

Atypical pneumonia is a form of pneumonia caused by atypical pathogens such as Mycoplasma pneumoniae, Chlamydophila pneumoniae, and Legionella pneumophila. It is also called 'walking pneumonia' due to its mild course that does not confine the patient to bed.

Unlike typical pneumonia, it begins with a flu-like presentation (headache, weakness, sore throat); fever and cough are added later. Findings on chest X-ray may be more severe than the patient's clinical appearance.

Beta-lactam antibiotics are ineffective against these pathogens; macrolides, quinolones, or tetracyclines are preferred.

Symptoms

Dry, persistent cough
Mild fever and weakness
Headache and muscle aches
Sore throat (initially)
Radiological findings may be more severe than clinical picture
Skin rash (rare in Mycoplasma)

Risk Factors

Young and school-age individuals (for Mycoplasma)
Crowded environments: school, barracks, dormitory
Autumn and winter seasons
Immune system suppression
Smoking

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If cough exceeds 2 weeks and is accompanied by fever
  • If shortness of breath develops
  • For chest X-ray and PCR test
  • If no response is obtained 48-72 hours after starting antibiotic

Treatment Methods

Macrolide antibiotics (azithromycin, clarithromycin — first line)

Quinolones or doxycycline (in Mycoplasma resistance or allergy)

Outpatient treatment is generally sufficient

Rest, plenty of fluids, and symptomatic treatment

Hospital admission in severe cases or immunocompromised patients
