What is Advanced Lung Cancer Screening with Low-Dose CT?

Low-dose computed tomography (LDCT) screening reduces lung cancer mortality in high-risk current and former heavy smokers when delivered as an annual structured program.

Eligibility typically includes adults aged 50-80 with a heavy smoking history (e.g., 20 pack-years or more) who are current smokers or quit within the past 15 years; advanced programs incorporate risk-prediction models and shared decision-making.

Findings are reported using the Lung-RADS system, with management algorithms ranging from continued annual screening for negative scans to short-interval follow-up, biopsy, or surgical resection for suspicious nodules; quality programs link screening with smoking cessation, incidentaloma management, and multidisciplinary review.

Symptoms

Most candidates are asymptomatic
Persistent cough in long-term smokers
Hemoptysis warranting separate evaluation
Unintentional weight loss
Chronic dyspnea
Recurrent pneumonia in same lobe
Chest pain or hoarseness in advanced disease

Risk Factors

Heavy and long-term tobacco smoking
Family history of lung cancer
Occupational exposures (asbestos, radon, silica)
Chronic obstructive pulmonary disease
Pulmonary fibrosis
Prior thoracic radiation therapy
Indoor air pollution and biomass smoke

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Heavy smoker considering screening
  • Suspicious nodule on a prior CT or chest x-ray
  • Family history of early lung cancer
  • Persistent respiratory symptoms in smoker
  • Hemoptysis or unexplained weight loss
  • Need for smoking cessation support
  • Lung-RADS 3 or 4 findings on screening

Treatment Methods

01
Eligibility assessment and shared decision-making
02
Annual low-dose chest CT in eligible adults
03
Lung-RADS structured reporting
04
Short-interval CT or PET-CT for indeterminate nodules
05
Biopsy or surgical resection for suspicious lesions
06
Integrated smoking cessation counseling and pharmacotherapy
07
Multidisciplinary review and tracking of program outcomes
