What is Advanced Complications of Asbestosis?

Asbestosis is interstitial lung fibrosis caused by inhalation of asbestos fibers, with a long latency from exposure to clinical disease.

Advanced complications include progressive interstitial fibrosis, secondary pulmonary hypertension, respiratory failure, malignant mesothelioma of pleura or peritoneum, and a markedly increased risk of lung cancer especially in smokers.

Management requires multidisciplinary care including occupational medicine, pulmonology, oncology and palliative services, with strict avoidance of further fiber exposure.

Symptoms

Progressive exertional dyspnea
Persistent dry cough
Bilateral basal crackles
Chest pain from pleural disease
Weight loss with mesothelioma
Recurrent pleural effusion
Cyanosis in advanced disease

Risk Factors

Cumulative occupational asbestos exposure
Long latency since first exposure
Concomitant cigarette smoking
Family member secondhand exposure
Exposure during demolition or renovation
Pre-existing pleural plaques
Genetic susceptibility to mesothelioma
