What is Advanced Management of Alveolar Hemorrhage?

Diffuse alveolar hemorrhage (DAH) is the diffuse filling of alveolar spaces with blood due to various causes including pulmonary vasculitis, Goodpasture syndrome, systemic lupus, antiphospholipid syndrome, drugs, and infections.

Clinically it is characterized by hemoptysis, rapidly progressive dyspnea, hypoxemia, and diffuse ground-glass opacities on CT. A rapid drop in hematocrit and a progressively bloodier bronchoalveolar lavage support the diagnosis.

Management includes rapid diagnosis and identification of the underlying disease, high-dose corticosteroids, immunosuppressive agents such as cyclophosphamide or rituximab, and in severe cases plasmapheresis and intensive care support.
