What is Acute Cor Pulmonale?

Acute cor pulmonale is a presentation that develops when the right ventricle encounters high afterload as a result of sudden disruption of pulmonary circulation. The most common cause is massive pulmonary embolism. ARDS and pneumothorax can also lead to a similar clinical picture.

The right ventricle cannot adapt to suddenly developed high pressure; dilation, increased wall tension, and loss of function develop rapidly. Left heart preload decreases and systemic circulatory collapse may occur.

Emergency diagnosis and treatment are essential. Echocardiography plays a critical role in rapid diagnosis. Emergency treatment of the underlying cause (thrombolysis, embolectomy, chest tube) is life-saving.

Symptoms

Sudden severe shortness of breath
Chest pain
Hypotension and shock
Syncope
Jugular venous distension
Right heart strain ECG findings
Cyanosis

Massive pulmonary embolism
Postoperative immobilization
Deep vein thrombosis
Malignancy
Severe ARDS
Tension pneumothorax
Hypercoagulability
