What is Venous Ulcer?

Venous ulcers are chronic, slow-healing wounds that typically develop around the medial malleolus as a result of chronic venous insufficiency and venous hypertension. They account for approximately 70% of lower limb ulcers.

Venous hypertension arises from superficial or deep venous valvular incompetence, superficial vein thrombosis or post-thrombotic syndrome following deep vein thrombosis. The surrounding skin shows lipodermatosclerosis and hyperpigmentation.

Diagnosis is based on clinical assessment and venous Doppler ultrasound. The cornerstone of treatment is compression stockings or bandages, wound care, skin protection and — in appropriate cases — superficial venous intervention.

Symptoms

Shallow ulcers around the medial malleolus
Leg edema
Brown hyperpigmentation
Lipodermatosclerosis (hardened skin)
Itching and eczema
Heaviness and pain in the leg
Exudation around the ulcer

Risk Factors

Chronic venous insufficiency
History of deep vein thrombosis
Obesity
Prolonged standing
Pregnancy
Advanced age
Family history of venous disease

  • Expanding ulcer or signs of infection
  • Increasing edema and pain
  • Recurrent venous ulcer
  • Established post-thrombotic syndrome
