What is Evaluation of Troponin Elevation?

Cardiac troponin (cTnI or cTnT) is the most specific biochemical marker of myocardial injury. High-sensitivity troponin (hs-cTn) tests can detect damage even at very low concentrations.

Troponin elevation has many causes other than acute MI: myocarditis, PE, sepsis, heart failure, renal failure, takotsubo and cardiac contusion. With serial measurements, the rise-and-fall pattern (delta change) is critical in distinguishing MI from non-MI.

Symptoms

Chest pain (typical or atypical)
Shortness of breath
Syncope
Sepsis or critical illness presentation
Heart failure findings

Risk Factors

Acute coronary syndrome
Myocarditis
Pulmonary embolism
Sepsis and septic shock
Chronic kidney disease (chronic low-level elevation)
Heart failure decompensation

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When troponin elevation is detected together with chest pain
  • Significant delta change on serial measurements (rise or fall)
  • Troponin elevation in a symptomatic patient
  • Cardiology consultation for unexplained troponin elevation
