What is Tricuspid Regurgitation?

Tricuspid regurgitation is a valve disease in which the tricuspid valve — between the right atrium and right ventricle — does not close completely, causing blood to flow back into the right atrium.

Most cases are functional (secondary), arising from right ventricular dilation secondary to left heart disease, pulmonary hypertension, or right heart failure. Rheumatic disease, infective endocarditis, and pacemaker leads can cause primary tricuspid regurgitation.

Elevated venous pressure leads to fluid accumulation in the systemic circulation. Mild cases generally require follow-up; in severe cases, surgical repair or replacement is considered.

Symptoms

Swelling in the legs and abdomen (edema and ascites)
Fatigue and exercise intolerance
Liver enlargement and right upper abdominal pain
Distended neck veins
Palpitations and atrial fibrillation
Loss of appetite and indigestion

Risk Factors

Left heart failure and mitral valve disease
Pulmonary hypertension
Infective endocarditis (especially in intravenous drug users)
Rheumatic fever
Carcinoid tumor

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When increasing leg swelling or abdominal distension is noticed
  • If symptoms worsen in someone with known heart disease
  • When jaundice or liver enlargement is noted
  • For cardiology evaluation with echocardiography and valve assessment

Treatment Methods

01
Treatment of the underlying cause (left heart disease, pulmonary hypertension)
02
Diuretics to reduce fluid overload
03
Tricuspid annuloplasty (valve repair — the preferred approach)
04
Valve replacement (when repair is not feasible)
05
Transcatheter tricuspid interventions (in high-risk elderly patients)
