What is TEVAR in Aortic Dissection?

Acute type B aortic dissection begins distal to the left subclavian artery. It is often managed medically, but intervention is required when complicated (malperfusion, uncontrolled pain, rapid aortic growth, rupture).

Thoracic endovascular aortic repair (TEVAR) uses a stent graft delivered through the femoral artery to seal the proximal entry tear, preserve the true lumen and promote thrombosis of the false lumen. It is preferred in complicated acute and subacute dissection.

TEVAR requires follow-up for risks such as paraplegia, retrograde type A dissection, graft infection and endoleak. Lifelong surveillance is mandatory after the procedure.

Symptoms

Sudden severe back or chest pain
Hypertensive emergency
Malperfusion (bowel, kidney, limb)
Paraplegia
Rapid hemodynamic decline
Neurologic findings
Loss of peripheral pulses

Risk Factors

Uncontrolled hypertension
Connective tissue disease (Marfan, Loeys-Dietz)
Bicuspid aortic valve
Prior aortic surgery
Trauma
Aortic aneurysm
Genetic predisposition
