What is TAVI Prosthetic Valve Dysfunction?

Transcatheter aortic valve implantation (TAVI/TAVR) has become the standard of care for aortic stenosis patients at high surgical risk. Over time, the prosthesis may develop structural valve deterioration, leaflet thickening, thrombosis, paravalvular leak or endocarditis.

Structural valve deterioration typically develops over 5–10 years and presents as restenosis or regurgitation. Other mechanisms include subclinical leaflet thrombosis (HALT), infective endocarditis and stent frame fracture.

Treatment options include anticoagulation, redo surgical valve replacement and — most commonly — valve-in-valve TAVI (a second transcatheter valve implanted inside the existing prosthesis).

Symptoms

Recurrent dyspnea
Reduced exercise capacity
Syncope or presyncope
Anginal chest pain
New heart failure signs
Palpitations
Fever suggesting endocarditis

Risk Factors

Advanced age
Long-standing TAVI prosthesis
Small prosthesis size
Prosthesis thrombosis
Hemodialysis
Diabetes and chronic kidney disease
Previous endocarditis

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • New dyspnea after TAVI
  • Anginal chest pain
  • New murmur
  • Unexplained fever
  • Exercise intolerance
  • Edema or syncope
