Takotsubo-Related Stress Cardiomyopathy

What is Takotsubo-Related Stress Cardiomyopathy?

Takotsubo cardiomyopathy (stress cardiomyopathy, broken heart syndrome) is a reversible myocardial dysfunction triggered by severe emotional stress (bereavement, fear, conflict) or physical stress (severe illness, surgery, intensive care). It produces a characteristic apical ballooning of the left ventricle.

The clinical picture mimics acute myocardial infarction: chest pain, dyspnea, ECG changes (ST elevation, T inversion) and elevated troponin. However, coronary angiography shows no obstructive disease, and apical ballooning on ventriculography is characteristic.

Catecholamine surge and microvascular dysfunction are considered the main mechanisms, and the great majority of patients are post-menopausal women. Treatment is supportive — heart failure care, beta-blockers, ACE inhibitors/ARBs and, when needed, therapy for LV outflow obstruction. Full recovery within 4–8 weeks is typical.

Pheochromocytoma
Acute asthma exacerbation
Intracranial hemorrhage
