Takotsubo Cardiomyopathy (Stress Cardiomyopathy)

What is Takotsubo Cardiomyopathy (Stress Cardiomyopathy)?

Takotsubo cardiomyopathy (stress cardiomyopathy, 'broken-heart syndrome') is a transient condition that develops after intense emotional or physical stress and typically produces apical akinesis of the left ventricle.

It is attributed to catecholamine-mediated transient myocardial injury. The symptoms and some ECG changes mimic a heart attack; however, no significant occlusion is seen in the coronary arteries.

It is markedly more common in postmenopausal women. Emotionally charged events (grief, intense arguments) or physical stressors (surgery, serious illness) can trigger it. Most patients recover fully within weeks.

Symptoms

Sudden chest pain after intense stress
Shortness of breath
ECG changes and elevated troponin
Palpitations and near-fainting
Signs of acute heart failure (in severe cases)
Cardiogenic shock (a rare complication)

Risk Factors

Postmenopausal female sex
History of neurologic or psychiatric disease
Intense emotional stress (grief, argument, surprise)
Severe physical illness, surgery, or trauma
Elevated catecholamine levels

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Go to the emergency department for sudden chest pain or shortness of breath after stress
  • Follow-up after heart attack has been excluded by angiography
  • For follow-up echocardiography within a few weeks
  • Cardiology follow-up for recurrence and complications

ACE inhibitor and beta-blocker (supportive of cardiac function during recovery)
