What is Superficial Thrombophlebitis?

Superficial thrombophlebitis is inflammation of a superficial vein with thrombus formation inside it. It most commonly affects the saphenous system in the lower limb, but upper-limb catheter-related and chest wall (Mondor) forms also exist.

Clinically it causes local pain, redness, warmth and a firm palpable cord along the vein. Lesions close to the saphenofemoral junction or extending over a long segment carry a higher risk of progression to deep vein thrombosis.

Diagnosis is based on clinical examination and venous Doppler ultrasound. Treatment includes NSAIDs, local measures, compression and, in selected cases, fondaparinux or low-dose anticoagulation.

Symptoms

Painful cord along the vein
Skin redness and warmth
Firmness and a palpable vein
Low-grade fever
Local swelling
Tenderness
Recurrent migratory lesions

Risk Factors

Varicose veins
Prolonged immobilization
Intravenous catheters
Pregnancy and postpartum period
Obesity
Thrombophilia
Malignancy (migratory thrombophlebitis)

  • High fever or signs of spreading infection
  • Rapid progression of symptoms
  • Lesion close to the saphenofemoral junction
  • Recurrent lesions concerning for malignancy
