Substrate Ablation for Ventricular Tachycardia

What is Substrate Ablation for Ventricular Tachycardia?

Ventricular tachycardia (VT) substrate ablation is an advanced electrophysiology procedure used primarily in patients with scar-related monomorphic VT from ischemic or non-ischemic cardiomyopathy. Three-dimensional electroanatomic mapping is used to survey the endocardium of the right and/or left ventricle and, when needed, the epicardium.

Low-voltage areas within scar and border zones, late potentials and local abnormal ventricular activity (LAVA) are annotated to identify reentrant circuits and ablated with radiofrequency energy. The procedure ends when induction testing no longer triggers the clinical VT.

Substrate ablation can be life-saving in patients with recurrent ICD shocks, drug-refractory VT and electrical storm.

Symptoms

Palpitations and non-sustained VT
Syncope or presyncope
Recurrent ICD shocks
Chest pain
Dyspnea
Fatigue
Worsening heart failure

Risk Factors

Prior myocardial infarction
Advanced structural cardiomyopathy
Pre-existing ICD
Failure of antiarrhythmic therapy
Electrical storm
Low ejection fraction
Specific substrates such as sarcoidosis or ARVC

  • New-onset sustained palpitations

  • VT recurrence on antiarrhythmic therapy
