What is Stress Cardiac MRI for Ischemia?

Stress cardiac magnetic resonance imaging assesses myocardial perfusion and wall motion under stress induced by a vasodilator (adenosine, dipyridamole, regadenoson) or dobutamine.

The technique is radiation-free and is preferred in the diagnosis of coronary artery disease in patients with intermediate-to-high pretest probability. It can evaluate the functional significance of ischemic lesions, microvascular dysfunction and viability in a single study.

Thanks to its high spatial and temporal resolution, it detects even small subendocardial defects. It carries a class I indication in guidelines, supported by trials such as SCOT-HEART and MR-INFORM.
