What is Sinus Node Reentrant Tachycardia?

Sinus node reentrant tachycardia is an uncommon paroxysmal supraventricular tachycardia caused by a reentrant circuit within or near the sinus node. The heart rate is usually between 100 and 150 bpm, and the P-wave morphology closely resembles normal sinus rhythm.

The arrhythmia starts and stops abruptly (paroxysmal behavior), which distinguishes it from physiological sinus tachycardia. Physiological sinus tachycardia slows gradually when triggers resolve, whereas sinus node reentry terminates suddenly.

Diagnosis is confirmed by electrophysiology study. Beta-blockers, calcium channel blockers and other antiarrhythmics can be used; catheter ablation is considered in symptomatic drug-refractory cases.

Symptoms

Palpitations with sudden onset and offset
Regular rapid heart rate
Dizziness
Fatigue
Chest discomfort
Dyspnea
Exercise intolerance

Risk Factors

Sinus node dysfunction
Coronary artery disease
Congenital heart disease
Older age
Electrolyte disturbances
Autonomic nervous system dysfunction
Previous cardiac surgery

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Palpitations with sudden onset and offset
  • Rapid palpitations unrelated to exertion
  • Palpitations with presyncope
  • Frequent recurrent symptoms
  • Episodes affecting quality of life
  • Suspicious ECG findings

Treatment Methods

01
Beta-blockers as first line
02
Calcium channel blockers (verapamil, diltiazem)
03
Antiarrhythmic drugs (flecainide, propafenone)
04
Vagal maneuvers during acute episodes
05
Catheter ablation in refractory cases
06
Avoidance of triggers
07
Correction of electrolyte imbalance
