Sinus Node Dysfunction (Sick Sinus Syndrome)

What is Sinus Node Dysfunction (Sick Sinus Syndrome)?

Sinus node dysfunction (sick sinus syndrome) is a chronic rhythm disorder caused by impaired impulse generation or conduction of the sinus node. It may present as sinus bradycardia, sinus pauses, sinoatrial block or tachy-brady syndrome.

Fibrosis of the sinus node is the main cause in older patients; ischemia, infiltrative diseases, autonomic dysfunction and drugs (beta-blockers, calcium channel blockers, digoxin) can also precipitate it.

Diagnosis is based on ECG, ambulatory rhythm monitoring and electrophysiology study. Permanent pacemaker implantation is the definitive therapy in symptomatic patients; anticoagulation may be needed given the risk of atrial fibrillation and thromboembolism.

Fatigue and weakness
Dizziness and lightheadedness
Syncope and presyncope
Palpitations in the tachy phase
Exercise intolerance
Confusion (in older adults)
Worsening heart failure symptoms

Risk Factors

Advanced age
Ischemic heart disease
Amyloidosis and infiltrative cardiomyopathy
History of cardiac surgery
Hypothyroidism
Autoimmune disease
Chronic drug therapy (beta-blockers, digoxin)

When to See a Doctor?

  • Syncope or presyncope
  • Marked fatigue and exercise intolerance
  • New tachy-brady symptoms
  • Pathological findings on ambulatory monitoring
  • Need for anticoagulation
