What is SGLT2 Inhibitors in Heart Failure?

SGLT2 inhibitors (dapagliflozin, empagliflozin) were initially developed for diabetes treatment but have been shown to reduce mortality and hospitalization in HFrEF in the DAPA-HF and EMPEROR-Reduced trials.

They are effective in heart failure patients with or without diabetes. The EMPEROR-Preserved trial also showed benefit in HFpEF. They act through natriuretic, diuretic, and cardioprotective mechanisms.

Symptoms

HFrEF (EF ≤40%) — NYHA II-IV
HFpEF (EF >40%) — symptomatic
Persistent symptoms despite optimal medical therapy

Risk Factors

Heart failure with or without diabetes
Concurrent chronic kidney disease (provides renal protection)
Recurrent heart failure hospitalizations

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • As part of four-pillar therapy in HFrEF diagnosis
  • When symptom control is inadequate in HFpEF
  • In concurrent diabetes and heart failure

Dapagliflozin 10 mg/day (DAPA-HF, DELIVER trials)

Empagliflozin 10 mg/day (EMPEROR-Reduced, EMPEROR-Preserved trials)

Indicated regardless of diabetes status

Add to standard HFrEF therapy (ACE-I/ARB/ARNI + beta-blocker + MRA)

Caution: hypovolemia, ketoacidosis, genitourinary infection risk

Renal function monitoring (effective up to eGFR 20 mL/min)
