What is SGLT2 Inhibitor Therapy in Heart Failure?

SGLT2 inhibitors (dapagliflozin, empagliflozin) were originally developed for type 2 diabetes but have become cornerstones of heart failure therapy following large clinical trials. They significantly reduce cardiovascular death and hospitalization in both HFrEF and HFpEF.

These drugs inhibit glucose reabsorption in the proximal renal tubule, increasing urinary glucose and sodium excretion. This diuretic-like effect reduces cardiac preload, suppresses neurohormonal activation and favorably affects myocardial metabolism.

Guidelines now include SGLT2 inhibitors with a class 1 recommendation in addition to standard therapy (ARNI/ACEI-ARB, beta-blocker, MRA) for symptomatic HFrEF.

Symptoms

Dyspnea
Foot and leg edema
Fatigue
Weight gain
Orthopnea
Paroxysmal nocturnal dyspnea
Exercise intolerance

Risk Factors

HFrEF (EF ≤ 40%)
HFpEF (EF ≥ 50%)
Diabetes mellitus
Chronic kidney disease
Prior cardiac event
High NT-proBNP
History of hospitalization

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • New or worsening heart failure signs
  • Weight gain and edema
  • Worsening dyspnea
  • Evaluation before starting SGLT2i
  • Urinary tract infection or dehydration
  • Signs of diabetic ketoacidosis

Treatment Methods

03
Beta-blocker titration
04
Combination with MRA
05
Diuretic therapy
06
Monitoring of renal function
07
Monitoring for urinary infection
