What is Reperfusion Strategies in Acute MI?

Early reperfusion therapy (restoration of coronary blood flow) is life-saving in ST-elevation myocardial infarction (STEMI). Primary percutaneous coronary intervention (PCI) is the preferred method, and door-to-balloon time should be kept below 90 minutes.

If PCI cannot be delivered within 120 minutes, fibrinolytic therapy is administered. Tenecteplase, alteplase and reteplase are commonly used fibrinolytic agents. Rescue or routine angiography is planned 2-24 hours after fibrinolysis.

Symptoms

Severe chest pain (>20 min, unresponsive to nitroglycerin)
Radiation to the left arm, jaw or back
Cold sweating and pallor
Nausea and vomiting
Dyspnoea
ST-segment elevation on ECG

Risk Factors

Smoking
Hypertension and diabetes
Hyperlipidaemia
Family history of premature CAD
Obesity and sedentary lifestyle
Stress

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Chest pain lasting more than 20 minutes — call 112 (EMERGENCY)
  • Chest pain with cold sweating and pallor
  • When ST elevation is detected on ECG
  • New chest pain in a patient with known coronary disease
