Raynaud's Disease

A vasospastic disorder in which the fingers and other acral areas change color in response to cold or stress.

What is Raynaud's Disease?

Raynaud's disease (Raynaud's phenomenon) is an exaggerated vasospastic response of the small arteries in the fingers, hands, feet, ears, or nose to cold or emotional stress.

A classic triphasic color change is seen: the skin first blanches (white), then turns cyanotic (blue), and finally becomes hyperemic (red). Each phase can last from minutes to hours.

Primary Raynaud's (not associated with an autoimmune or other underlying condition) is far more common and benign. Secondary Raynaud's is linked to systemic diseases such as scleroderma, SLE, or occupational vibration exposure, and may require further investigation.

Symptoms

Fingers first turning pale/white in the cold
Subsequent bluish-purple discoloration (cyanosis)
Redness and throbbing pain on rewarming
Numbness and tingling
A sensation of swelling in the fingertips
Rare severe cases — fingertip ulcers (in secondary Raynaud's)
Symptoms triggered by cold or stress

Risk Factors

Female sex (3:1 more common)
Younger age (typical for primary Raynaud's, 15–30 years)
Exposure to cold climates
Scleroderma, SLE, rheumatoid arthritis (secondary Raynaud's)
Work with vibrating tools
Smoking
Medications such as beta-blockers

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If fingertip ulcers or non-healing wounds appear
  • If symptoms are progressively worsening and do not resolve with rewarming
  • Accompanying joint pain, dry eyes/mouth, or rash (to investigate for autoimmune disease)
  • At the first episode, to establish the diagnosis and exclude secondary causes
  • For treatment options when symptoms significantly impair quality of life

Cold avoidance: gloves, heated socks, and layered clothing

Smoking cessation and avoiding sudden cold exposure

Calcium-channel blockers (nifedipine is effective in reducing vasospasm)

Topical nitroglycerin (for fingertip ulcers)

Phosphodiesterase inhibitors (in severe cases)
