What is Quantitative Myocardial Blood Flow by PET?

Quantitative myocardial blood flow assessment by PET uses radiotracers such as rubidium-82 or 13N-ammonia to measure absolute MBF (mL/g/min) at rest and stress.

Coronary flow reserve (CFR < 2.0) and global/regional MBF values are valuable for detecting microvascular dysfunction, multivessel CAD and cardiac allograft vasculopathy.

The technique has high diagnostic and prognostic value; it is especially valuable in microvascular angina, post-transplant follow-up and multivessel disease.

Symptoms

Microvascular angina
Chest pain with open coronary arteries
Post-transplant evaluation
Suspected multivessel disease
Atypical ischemic symptoms
Chest pain in diabetes
Unclear symptoms in women

Risk Factors

Suspected microvascular dysfunction
Heart transplantation
Multiple risk factors
Diabetes mellitus
Previous inconclusive non-invasive test
Chronic kidney disease
Unexplained chest pain

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Suspected microvascular angina
  • Allograft vasculopathy follow-up in transplant
  • Multivessel evaluation
  • Insufficient previous non-invasive test
  • Prognostic evaluation
  • High-risk patient workup

Treatment Methods

01
Microvascular therapy (calcium channel blockers, ranolazine)
02
Standard CAD management
03
Revascularization when indicated
04
Optimization of immunosuppression in transplant
05
Aggressive risk factor control
06
Follow-up PET
07
Multidisciplinary approach
