Pulmonary Vein Isolation (Cryoablation)

What is Pulmonary Vein Isolation (Cryoablation)?

Pulmonary vein isolation (PVI) is a catheter ablation technique that electrically separates the left atrium from the pulmonary vein ostia, which harbor the triggers for atrial fibrillation (AF). In cryoablation a balloon catheter is positioned at the pulmonary vein ostium and cooled to −40 to −60 °C, producing a durable fibrotic lesion.

Cryoablation has become a widely used alternative to radiofrequency ablation for paroxysmal AF and has shortened procedure times. Guidelines recommend ablation as a class 1 indication in symptomatic drug-refractory patients.

The procedure is performed under fluoroscopy and sometimes intracardiac echocardiography. One-year AF-free survival after a successful isolation reaches 70–80% in selected patients.

Drug-refractory paroxysmal atrial fibrillation
Early-ablation candidates without structural heart disease
Symptomatic AF episodes

Sleep apnea
Obesity
Hyperthyroidism
