Pulmonary Artery Denervation

What is Pulmonary Artery Denervation?

Pulmonary artery denervation (PADN) is a newer percutaneous technique that uses radiofrequency energy to ablate sympathetic nerve endings in the pulmonary artery wall. The goal is to reduce increased sympathetic activity, lower pulmonary vascular resistance and relieve right ventricular afterload.

The procedure is performed during right heart catheterization, with an ablation catheter positioned at the main pulmonary artery bifurcation and main branches. Early studies have reported hemodynamic and functional improvement in patients with Group 1 pulmonary arterial hypertension (PAH) and selected Group 2 patients.

The method remains investigational; it is not a routine guideline-recommended therapy and is used in selected cases within clinical trials.

Exercise intolerance
Fatigue
Dizziness and syncope
Chest pain
Leg edema
Abdominal distension

Risk Factors

Group 1 pulmonary arterial hypertension
Drug-refractory PH
High pulmonary vascular resistance
Right ventricular dysfunction
Connective tissue disease
Congenital heart disease
Idiopathic PAH

  • Worsening cardiac MRI or right heart catheterization parameters
