Postural Orthostatic Tachycardia Syndrome (POTS)

What is Postural Orthostatic Tachycardia Syndrome (POTS)?

Postural orthostatic tachycardia syndrome (POTS) is an autonomic nervous system disorder defined by an increase of ≥30 beats per minute in heart rate with symptoms upon standing from the supine position.

Symptoms develop on standing and resolve with lying down or sitting. Increased sympathetic tone and inadequate venous return underlie the condition.

Commonly seen in young women, POTS has become markedly more prevalent after COVID-19. Diagnosis is confirmed with a tilt-table test. Although benign, it can significantly impair quality of life.

Symptoms

Dizziness and lightheadedness on standing
Rapid palpitations
Fatigue and low energy
'Brain fog' (difficulty concentrating)
Near-fainting or a tendency to faint
Nausea and abdominal pain

Risk Factors

Young female sex (15–50 years)
Prolonged bed rest or immobility
Following a viral illness (including COVID-19)
Autoimmune disease
Ehlers–Danlos syndrome and connective tissue disorders

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If palpitations, lightheadedness, or fainting occur on standing
  • Unexplained fatigue and brain fog with tachycardia
  • If these symptoms develop after COVID-19
  • For tilt-table testing and autonomic assessment with cardiology/neurology

Treatment Methods

Increased salt and fluid intake (2–3 L of water daily with adequate sodium)

Compression stockings and abdominal binders (to improve venous return)

Regular exercise (particularly recumbent programs)

Beta-blockers, fludrocortisone, or midodrine (if medication is required)

Multidisciplinary rehabilitation in COVID-related POTS
