Postural (Orthostatic) Hypotension

What is Postural (Orthostatic) Hypotension?

Postural hypotension is defined as a drop in systolic blood pressure of at least 20 mmHg and/or diastolic of at least 10 mmHg on standing from a lying or sitting position. This reduces cerebral blood flow and can cause dizziness, blurred vision and syncope.

It is classified as neurogenic (due to autonomic dysfunction) or non-neurogenic (dehydration, medications, bleeding). Prevalence may reach 20% in older adults and significantly increases fall risk.

Diagnosis is made by comparing blood pressure measured while supine and again 1–3 minutes after standing. A tilt-table test provides more detailed assessment.

Initial management focuses on treating underlying causes and lifestyle changes; pharmacologic therapy is added for refractory cases.

Symptoms

Dizziness or lightheadedness on standing
Blurred or darkened vision
Syncope or presyncope
General weakness and fatigue
Nausea and sweating
Risk of fall-related injury

Risk Factors

Age over 65
Use of antihypertensives, diuretics and antidepressants
Parkinson disease and autonomic neuropathy
Diabetic neuropathy
Prolonged bedrest and dehydration
Amyloidosis and multiple system atrophy
