What is Post-Thrombotic Syndrome?

Post-thrombotic syndrome (PTS) is chronic venous insufficiency that develops in about one third of patients after a deep vein thrombosis. Venous hypertension, valvular incompetence and residual obstruction are the main mechanisms.

It presents with leg pain, swelling, heaviness, skin changes and, in severe cases, venous ulcers. The Villalta score is used to grade severity, and the condition significantly impairs quality of life.

Treatment includes compression stockings, exercise, wound care and, when needed, venous stenting or thrombolysis. Effective early anticoagulation for DVT reduces the risk of PTS.

Symptoms

Chronic leg pain
Edema and heaviness
Hyperpigmentation and varicose veins
Lipodermatosclerosis
Itching and eczema
Venous ulcer
Activity limitation

Risk Factors

Proximal deep vein thrombosis
Recurrent DVT episodes
Obesity
Inadequate anticoagulation
Advanced age
Pre-existing venous disease
Poor adherence to compression therapy

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Persistent leg swelling and pain after DVT
  • Skin and color changes
  • Development of a non-healing wound
  • Significant reduction in quality of life
  • Unresponsive to compression therapy

Treatment Methods

01
Compression stockings (30–40 mmHg)
02
Regular walking and leg exercises
03
Weight control
04
Appropriate wound care for venous ulcers
05
Venous stenting or recanalization in selected cases
06
Indications for long-term anticoagulation
