What is Post-Myocarditis Dilated Cardiomyopathy?

Approximately 10-20% of acute myocarditis cases progress to chronic dilated cardiomyopathy. Persistent inflammation and autoimmune mechanisms maintain myocardial damage and lead to progressive left ventricular dilation and reduced ejection fraction.

Endomyocardial biopsy evaluates active inflammation and fibrosis. Late gadolinium enhancement on cardiac MRI shows the extent of fibrosis and has prognostic significance.

Symptoms

Progressive shortness of breath
Exercise intolerance
Peripheral edema
Palpitations and arrhythmias
Syncope (sudden cardiac death risk)

Risk Factors

Persistent low EF after fulminant myocarditis
Extensive late gadolinium enhancement on cardiac MRI
Giant cell myocarditis
Viral persistence (PCR positivity on endomyocardial biopsy)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When EF has not recovered after myocarditis
  • Progressive heart failure symptoms
  • Development of ventricular arrhythmias
