Post-MI Ventricular Septal Rupture

A mechanical complication of acute myocardial infarction in which the interventricular septum ruptures.

What is Post-MI Ventricular Septal Rupture?

Post-MI ventricular septal rupture occurs when necrotic septal tissue perforates after acute myocardial infarction. It typically presents in the first week following MI. Its incidence has fallen with widespread reperfusion therapy, but mortality remains very high.

Defects after anterior MI are usually located in the apical septum, whereas those after inferior MI tend to involve the basal posterior septum. The rupture creates an acute left-to-right shunt from the left ventricle to the right ventricle and imposes right heart overload.

Diagnosis is confirmed by urgent echocardiography. Treatment is surgical closure or, in selected cases, percutaneous device closure. Mechanical circulatory support may be used for preoperative stabilization.

Symptoms

New pansystolic murmur after MI
Sudden deterioration and cardiogenic shock
Signs of right-sided heart failure
Jugular venous distension
Hypotension
Pulmonary edema
Low cardiac output findings

Large transmural MI
Delayed reperfusion
Advanced age
Female sex
Hypertension
Single-vessel disease
First myocardial infarction
