What is Post-CABG Complications?

A variety of complications may develop in the early and late period after coronary artery bypass grafting (CABG). Early complications include atrial fibrillation (25–40%), low cardiac output syndrome, perioperative myocardial infarction, sternal wound infection, pulmonary complications and neurocognitive disturbances.

Late complications include graft occlusion, aneurysm formation, sternal dehiscence, post-pericardiotomy syndrome and depression. Risk factors include advanced age, diabetes, COPD, low ejection fraction, urgent surgery and postoperative renal dysfunction.

Prevention and treatment are multifaceted: beta-blocker prophylaxis for atrial fibrillation, aggressive pulmonary physiotherapy, rigorous infection control, early mobilization and cardiac rehabilitation reduce complications. High-risk patients require intensive care and multidisciplinary follow-up.

Symptoms

Palpitations from atrial fibrillation
Dyspnea
Discharge or swelling at the sternum
Fever (infection)
Chest pain
Leg wound swelling at the saphenous harvest site
Cognitive impairment and depression

Risk Factors

Advanced age
Diabetes and renal failure
Low ejection fraction
Urgent surgery
Use of bilateral internal mammary artery
Obesity and COPD
Long cardiopulmonary bypass time

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Discharge or swelling at the sternum
  • New palpitations after surgery
  • Unexplained dyspnea
  • Signs of leg wound infection
  • Fever
  • Worsening cognitive status after surgery
