What is Post-Angioplasty Care?

Coronary angioplasty (PTCA) is the opening of a narrowed or occluded coronary artery with a balloon and/or stent. In patients who receive a stent, the most critical period is while the body is adjusting and a new endothelial layer is forming over the metal surface.

The most important risk during this period is clot formation within the stent (stent thrombosis). To prevent this, dual antiplatelet therapy with aspirin and a P2Y12 inhibitor (clopidogrel, ticagrelor, or prasugrel) is essential for 6–12 months or longer.

To improve quality of life and prevent further events, regular cardiology follow-up and comprehensive lifestyle modification after angioplasty are vitally important.

Symptoms

What to expect during normal recovery after angioplasty:
Mild tenderness at the groin or wrist if that was the access site
Mild fatigue in the first few days
Warning symptoms (seek emergency care): recurrent chest pain
Sudden shortness of breath or sweating
Swelling or bleeding at the groin/wrist access site

Risk Factors

Situations that increase the risk of stent thrombosis:
Non-adherence to antiplatelet medications (self-discontinuation)
Diabetes and renal failure
Small-caliber stent or long stent
Bifurcation lesions
Stents placed during an acute heart attack

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Schedule a routine cardiology visit at 1 month after the procedure
  • Go to the emergency department immediately for any chest pain
  • Consult your physician before stopping antiplatelet drugs (for surgery, etc.)
  • If swelling or bruising at the access site is spreading
  • If new shortness of breath or palpitations develop

Treatment Methods

01
Dual antiplatelet therapy: aspirin plus a P2Y12 inhibitor (clopidogrel, ticagrelor)
02
Statin therapy (plaque stabilization and LDL control)
03
Beta-blockers and ACE inhibitors (to protect the left ventricle)
04
Smoking cessation, Mediterranean diet, and regular exercise
05
Cardiac rehabilitation program (6–12 weeks)
06
Annual stress test or imaging to check stent patency
